Repeat Prescriptions

Please allow at least 2 working days for us to deal with your repeat Prescription request.

To make a request for a repeat prescription by hand either:

• Complete a Prescription request form available from the surgery and give it to us
• Deliver the retained part of your computerised Prescription to the surgery

Alternatively you can send it electronically to us by email or fax (01293 450401). Please include the following information when electronically sending a request:

• Patient name
• DOB
• Address
• Name of requested medication
• Where you would like to pick it up from (It is possible to avoid a further visit to the surgery by collecting your completed prescription direct from a local chemist)

Please note we do not accept Prescription requests over the telephone and if you request medication that has not been given to you on a regular basis, the Doctor may need to see you.
